A successful transition process is based on the student strengths, preferences and interests, and requires collaboration between the student, family, school district and community agencies. Transition planning and services are designed to prepare a student with a disability to achieve his or her post-secondary goals related to living, learning and earning within the community.
In this training you will learn what is required by educational entities in planning and preparing for a student’s transition from high school to adult life. Parents and other participants will also be given the opportunity to:
- Identify key actions that should take place during a young adults’ journey to adult life in order to be best prepared;
- review necessary components of the IEP for Post-Secondary Transition planning
- become familiar with various tools and assessments that will strengthen the transition process and assure preparatory activities are targeted and purposeful
Parents will be given a comprehensive resource binder which will include general information, regulation requirements and transition planning tools.
